Cook County Clerk Elections is pleased to introduce our new website for Permanent Vote By Mail.

You will have the option to make any changes to your permanent vote by mail status, instantly online.

Some of the changes include:

• Make party primary ballot preference changes.
• Opt-in/Opt-out of permanent vote by mail.

You can do this all online by clicking here.  If needed, you will also be able to download a paper version of the Permanent Vote By Mail Opt-in/Opt-out form that you can complete and return to the Cook County Clerk’s Mail Ballot Department.
